The Granite Ridge Retirement Residence in Gravenhurst is providing five transitional care beds to the Muskoka Algonquin Healthcare (MAHC).
According to officials, the joint partnership will assist community patients who no longer need acute hospital treatment but require further rehabilitation and support before returning home or moving to a long-term care setting.
“Transitional beds help patients continue their recovery in a more appropriate setting while allowing our hospitals to focus on delivering acute care to those who need it most,” said Cheryl Harrison, President and CEO of Muskoka Algonquin Healthcare.
“This collaboration reflects our commitment to working with community partners to improve access, flow, and patient experience.”
Jackie Payne, Director of Operations at Granite Ridge Retirement Residence, said they are honoured to partner with Muskoka Algonquin Healthcare to support the introduction of transitional care beds at Granite Ridge.
“These beds will allow individuals to continue their recovery and rehabilitation in a supportive environment while helping to ensure hospital resources are available for those who need acute care,” said Payne.
According to officials, this new partnership is timely, following the scheduled conclusion of the transitional bed agreement with Hospice Muskoka (Andy’s House) where their transitional beds used to be.
Donna Kearney, Executive Director of Hospice Muskoka, said they will continue to support MAHC by accepting referrals to Andy’s House for end-of-life care.
Additionally, the two organizations will maintain their partnership through the Andy’s House at Home Program, which assists individuals receiving palliative care in their own homes.
“Our shared goal is a future where pain and symptom management can be provided to people in Muskoka and the surrounding area without the need for emergency room visits,” said Kearney.
